Ways to Order


IMPORTANT: Regardless of the way you place your order, please remember that a copy of your physician's original prescription must be provided to EPS to begin processing it. Prescription copies can be provided by mail, fax, by Emailing a scanned copy, or you can request that EPS contact your physician directly.


  • ORDER ONLINE:
    Click on the link below, fill out the electronic order form completely, then click the "Submit" button to send your order to the EPS Order Processing Team.

  • ORDER BY PHONE:
    If you are ordering for the first time, an EPS Representative will assist you in enrolling in the EPS Program.

    Download, print and complete the below order form. Be sure to complete both pages. Attach your prescription(s) or refill label(s) to the order form and mail completed form to:

Important Ordering Criteria


IMPORTANT: Regardless of the way you place your order, please remember that a copy of your physician's original prescription must be provided to EPS to begin processing it. Prescription copies can be provided by mail, fax, by e-mailing a scanned copy.


  • ORDER FORM:
    Ensure that the two page Order Form is completed; orders cannot be processed if the submitted order forms are missing information or information is not legible.

  • CO-PAYMENTS (If Applicable) and SHIPPING FEES:
    If your benefit plan requires that you make a co-payment or you have an unmet deductible, be sure your desired method of payment is indicated.
    • If co-payment is not received within 10 business days of order request, then fulfillment of your order will be terminated.
    • Be sure to add shipping fees to your co-payment amount when ordering.
    • If you are unsure of the amounts owed for co-payment and/or shipping fees, please contact EPS at eps@universalrx.comor call 540-777-1450 and an EPS Representative will be happy to assist you.

  • FORMS OF PAYMENT - Credit Cards:
    This is the preferred method because it is the quickest way for EPS to process.
    Cards Accepted by EPS:
    • Visa, MasterCard or American Express
    Be sure to provide all necessary information to ensure prompt processing:
    • Name as it appears on card
    • Billing address
    • Type of Card
    • 16-digit credit card number
    • Expiration Date

  • FORMS OF PAYMENT - Checking Account Debit (ACH):
    Be sure to provide all necessary information to ensure prompt processing:
    • Full Name of Accountholder
    • Routing Number
    • Account Number
    • Voided Check
    • Signature required on order form (This provides EPS authorization to process your payment from your checking account.)

  • FORMS OF PAYMENT - Check:
    Be sure to follow the below guidelines to ensure prompt processing:
    • Make check payable to: Universal Rx
    • Check must clear at the bank before order is processed, which may take up to 10 business days
    • Mail check with completed order and copy of original physician's prescription to:

  • FORMS OF PAYMENT - Money Order:
    Be sure to follow the below guidelines to ensure prompt processing:
    • Make Money Order payable to: Universal Rx
    • Send Money Order with completed order and copy of original physician's prescription to:

  • ORDER MINIMUMS:
    Due to the special nature of these orders, a minimum of a 90-day supply is required.
    • Ask your physician to prescribe a minimum of a 90-day supply.

  • YOUR SHIPPING ADDRESS:
    Ensure the shipping information is a physical address.
    • Orders cannot be shipped to Post Office Boxes.

EPS Order Timeline

  • INITIAL ORDERS:
    Please allow 4-5 weeks for the ordering process and delivery to take place on your initial order.

  • TYPICAL ORDER PROCESS TIMELINE:
    Once all enrollment information and prescriptions are provided, the process timeline will be as follows:
    • Within 24 business hours:
      Your assigned EPS Representative will contact you via e-mail or phone to provide confirmation that the fulfillment process has started.
    • Within 3 business days:
      Your paperwork will be processed and your order will be submitted to the pharmacy. The order process begins.
    • Business Days 4-6:
      Your order will be filled and shipped. Your assigned EPS Representative will provide you with the tracking and shipping information so that you can track the progress of delivery along with your assigned EPS Representative via the delivery courier's website.
    • Business Days 7-12:
      Your order will be in the process of being delivered to your location. Delivery time will vary based on location and customs requirements. Your assigned EPS Representative will remain in contact with you until you have provided confirmation that your package has been delivered.
    • This timeline is based on averages. Times may vary depending on specific circumstances affecting your order.

  • RE-ORDER REMINDER:
    Based on the amount of medication that you ordered, your assigned EPS Representative will contact you via e-mail 3 weeks prior to you needing your medication to remind you that a refill is due.


EPS Re-Orders

  • EPS WILL CONTACT YOU ON ALL RE-ORDERS:
    An EPS Representative will contact any members who have re-orders available on their original script(s).
    • Members will be contacted via e-mail or phone approximately 3-weeks prior to when their medication will be depleted.
    • Two attempts will be made to reach the member for authorization to process available refill(s).
    • If you have prescription refill(s) available and have not been contacted by an EPS Representative, please be sure to contact our office directly.
    • You can contact EPS by:

           3.  Fill out an Online Order Form

  • MINIMUM RE-ORDER NOTIFICATION:
    All refill requests must be received at least 14 days prior to the depletion of your current supply. This timeframe is needed to safely and accurately fill your prescription(s) and ship them to you in the required time necessary.
    • If re-order requests are received with less than a 14-day lead time, EPS will not be able to guarantee that your prescription medication will reach you before your current supply is depleted.
